1. Title page using APA format2. Grant Proposal Section 4- Logic Model: Complete 3 logic model tables (in the template)- one for each problem your group intends to address with your health program. 3. For each of the 3 logic model tables, complete all 6 columns. Complete the required column content as outlined below and in the grading rubric. Columns must match. o Resources column (inputs) includes specific human, financial, organization, and/or community resources needed to complete the health program’s activities. Include resources you already have at the clinic, as well as resources you need funding to obtain. Highlight the resources you need. Note: resources may be repeated in each of your 3 logic model tables- this is ok. o Activities column (outputs) includes specific events, actions, processes, and/or tools that will be completed when implementing your health program. o Participants column (outputs) includes the persons you will reach or involve with each of your health program’s activities. o Short-term column (outcomes) includes the specific immediate changes that are desired for health program participants. Short-term outcomes are often the changes in knowledge and/or attitudes that come immediately after the activities (such as educational activities) of a health program are implemented. Express using SMART format to ensure outcomes are measurable.
o Medium-term column (outcomes) includes the specific intermediate changes that are desired for health program participants. Medium-term outcomes are often the changes in behavior/actions and health status that occur within the first 1-3 years of a health program. Express using SMART format to ensure outcomes are measurable. o Long-term column (outcomes/impact) includes the specific ultimate changes that are desired for health program participants. Long-term outcomes are often the drastic changes in health status (of participants or community at large) that occur about 5+ years after the start of a health program. Express using SMART format to ensure outcomes are measurable.
